1. Who is AmeriPlan®?
AmeriPlan® is a Nationwide Discount Medical Plan Organization. We arrange for our members to have access to physicians, ancillary services, hospital advocacy, dental, vision, prescription drug and chiropractic providers who have agreed to offer their services at negotiated discounts off their usual and customary fees.
2. How long do I have to wait to use this program?
You don't have to wait at all. You can use this program as soon as you receive your membership cards - about 10 to 14 business days! And, you can use your benefits as much as you like. All preexisting conditions are accepted with no waiting periods (with the exception of orthodontics already in progress.)
3. How do I pay my Membership Fee?
Monthly or quarterly payments are made ONLY by electronic bank draft, debit card, or by credit card. INVOICING IS DONE ONLY FOR ANNUAL MEMBERSHIPS PAID ONE YEAR IN ADVANCE. An individual membership is $49.95 per month and a family membership is $59.95 per month. Family membership covers all residents in the household, including children, parents, relatives, and significant others.
4. How much is the deductible?
There is no deductible to reach before you receive the savings.
5. What are the Additional benefits that I get with this plan?

6. How much more do pharmacy, vision and chiropractic plans cost?
The dental care, prescription drug, vision, and chiropractic plans are absolutely FREE with the health plan membership.
7. Who are the Health, Dental, Pharmacy, Vision and Chiropractic Care Providers?
AmeriPlan Health® Care has assembled one of the largest networks of dental providers in the nation, and we are continually adding to this base. These dentists are well-established, with successful dental practices. Our prescription card may be used at our participating pharmacies nationwide, as well as through mail order. The vision care benefits are available nationwide at more than 12,000 providers, and our chiropractic benefits may be used with more than 7,000 credentialed chiropractors. You may search for dental providers in you area by entering your zip code into the field labeled Step 1 on the main page.
8. How do I locate an AmeriPlan Health® provider?
There are three ways to locate a provider. Instructions are included in the Member Information Guide that you will receive with your identification cards.They are:

* Call AmeriPlan Health® Customer Service at the listed number in your guide.
9. How do I use the Plan?
Within 10 to 14 working days of receipt of your application, you will receive a Member Information Guide, Membership Identification Cards and a Health, Dental, Pharmacy, Vision and Chiropractic Care Directory. Present your Identification Card to any AmeriPlan® provider and you will be eligible to get the discounted fees. You pay the provider at the time of service.
10. Will my family be covered?
When you enroll in the plan you have the option of choosing an individual plan for $49.95 per month or a family plan for $59.95 per month. Your family will only be covered by your plan if you have chosen the family plan.
11. Will I receive a card to show my providers?
Yes. They are a standard sized card -about the size of a credit card- with the member's name on it.
12. How long do I have to keep this program for? Is it a year-long contract or month-to-month?
It is a one year commitment and after the first year it is month-to-month.
13. Is there a guarantee with this plan?
YES! You have an unconditional 30-day money back guarantee. If you are unsatisfied for any reason within the first 30 days you will receive 100% of your money back.

The benefits with the dental plan are so good that we prefer to offer it in many cases over the dental insurance policies we have available and the health plan is a great alternative or supplement to traditional health insurance plans.
